Here's how to sign up now for Time2Pay®. It's fast and easy!

Step 1:

Download, complete and sign these forms:

  1.   Accuchex Client Master File
  2.   Accuchex Tax Service Agreement
  3.   Bank Authorization
  4.   Company Direct Deposit Agreement
  5.   Employee Direct Deposit Agreement
  6.   Laser Signature Form
  7.   Accuchex Service Summary
  8.   IRS Form 8655 Reporting Agent Authorization for Electronic Filers
  9.   IRS Form 8821 Tax Information Request Authorization
  10. CA Form DE48 Power of Attorney

Step 2:

If you are a new business, you may need to request your Employer ID# from your State and Federal IRS. Please download the forms or click on the links below to apply.

  IRS Form SS4 Employer ID Request    (Or)
  Federal Employer Identification number (EIN)

  CA Form DE1 State ID Request    (Or) 
  California Identification Number (DE1)

Your employees may need to complete a W-4 form and I-9 Form. Please click on the links below

  2010 W-4 Form
  2010 I-9 Form

Step 3:

Fax completed forms to 1-415-532-2806.

Make copies for your records and send originals to:
